AWAKE THE SOUL, LLCÂ - CLIENT SERVICE AGREEMENT
Welcome to the practice of Awake the Soul, LLC. This document contains important information about the products, services, and business policies provided by the company. When you sign this document, it represents an agreement between you and Awake the Soul, LLC., its brand offering: A Common Awakening, and its licensed affiliates and agents (hereinafter referred to as “Awake The Soul, LLC.”). I agree that I have been advised to contact a representative of Awake the Soul, LLC and/or my personal attorney to discuss any questions I may have prior to signing this document.
MY RIGHTS
As a client of Awake the Soul, LLC., I acknowledge that I have certain rights and responsibilities. These rights and responsibilities are described in the following sections.
- I am entitled to receive information about the methods of spiritual guidance, the techniques used, and the duration of spiritual guidance, if known, and the fee structure.
- If psychotherapy is desired, I will independently pursue the services of a licensed psychotherapist.
- If medical consultation is desired, I will independently pursue the services of a licensed medical provider.
- I may seek a second opinion from another provider and may terminate services at any time.
- In a professional relationship, sexual intimacy is never appropriate and should be reported to the Department of Regulatory Agencies, State of Colorado.
- Information provided during spiritual guidance sessions is confidential except for certain legal exception that will be identified by the provider should any such situation arise during spiritual guidance sessions. Situation exceptions may include but are not limited to child abuse, danger to oneself or others, etc.
INFORMED CONSENT
I acknowledge that Awake the Soul, LLC and its licensed affiliates do not offer medical, psychological or crisis services. I understand that the medical and/or counseling profession does not necessarily recognize or understand the spiritual work of Awake the Soul, LLC, and I also acknowledge Awake the Soul, LLC, and its licensed affiliates, will not be practicing psychotherapy or providing medical care. Therefore, I will continue to see my regular doctor and/or counselor and all medical or psychological decisions will be made explicitly between my health professional, counselor and me. I understand that I am fully responsible for communicating with the affiliates of Awake the Soul, LLC. regarding any special health needs, issues or concerns, psychologically or physically that may be sensitive to spiritual change. I understand that spiritual guidance, discussions, teachings, ceremonies, workshops, readings, retreats, and meditations may lead to some discomfort and disruption in my life.
I further understand that no results have been implied or guaranteed to me personally by Awake the Soul, LLC. and its licensed affiliates. By participating in spiritual guidance work with Awake the Soul, LLC, I acknowledge that I have been duly informed of the benefits and risks of spiritual guidance. I recognize that Awake the Soul, LLC will be sensitive and aware of each person’s spiritual needs to the best of its abilities, yet the organization cannot take responsibility for my own personal choices. I certify that to the best of my knowledge, I do not have any medical or psychological condition or any physical issues, which would prohibit me from participating in spiritual guidance, discussions, teachings, ceremonies, workshops, readings, retreats, and meditations.
Being 100% responsible for myself, I am open to receive and benefit in all ways from the services provided by Awake the Soul, LLC including, but not limited to, energy work, spiritual guidance, guided meditations, facilitated retreats and/or discussions with the affiliates of Awake the Soul, LLC.
CANCELLATION POLICIES
All offerings made by Awake the Soul, LLC. including, but not limited to, retreats, monthly packages, coaching sessions, classes/webinars and teaching session purchases are non-refundable. By purchasing the services offered through Awake the Soul LLC, I agree to these terms and conditions. Spiritual readings may be canceled greater than 24 hours in advance. If I miss a spiritual reading session without canceling, or cancel with less than 24 hour notice, I am responsible for the full payment of that session, without a refund [unless agreed upon ahead of time]. In addition, I am responsible for coming to my session on time; if I am late, my appointment will still need to end on time. All payments must be made in full prior to the beginning of a session.
PRIVACY POLICY
All information about confidentiality, as well as other information about my privacy rights, are fully described in a separate document on the Awake the Soul, LLC website at www.annielbrowning.com entitled Privacy Practices. If further clarifications are needed, I acknowledge that I will reach out to a representative of Awake the Soul, LLC.
TERMINATION OF SERVICES
This Agreement may be terminated at any time by either party upon written notice to the other party. In the event of such termination, I agree to comply with all cancellation policies detailed by Awake the Soul, LLC.
WARRANTIES
I understand that the programs and services offered by Awake the Soul, LLC. may accelerate my awakening journey. I acknowledge that each journey is unique. It is impossible to predict how that might look for me, and there may be unexpected consequences to this process. Consciousness expansion often involves a variety of experiences that can include a greater sense of peace, love, joy, centeredness, and new perspective on life while also involving uncomfortable feelings such as sadness, guilt, anxiety, anger, frustration, loneliness, and helplessness. This may be experienced because the process of spiritual growth often requires exploring the unpleasant aspects of my life and facilitate shifts in my overall experience. Spiritual growth has been shown to have benefits for individuals who undertake it but may also involve discomfort and life altering impact.
I agree that Awake the Soul, LLC., and its licensed affiliates, makes no explicit or implied warranties as to the usefulness or effectiveness of its products and services and that the results may vary depending on each individual. Furthermore, I have been advised to reflect on whether this process is a proper fit for me prior to beginning my engagement with Awake the Soul, LLC. I agree to absolve Awake the Soul, LLC, its licensed affiliates, shareholders, officers, employees and agents from all liability resulting from the use of any products and/or services provided by Awake the Soul, LLC.
INTELLECTUAL PROPERTY
I agree that all material provided by Awake the Soul, LLC is copyrighted and the exclusive property of Awake the Soul, LLC. This includes, but is not limited to audio/video recordings, as well as any and all materials associated with retreats, monthly packages, coaching sessions, classes/webinars and
sessions. I agree that I will not modify, forward, or generally alter in anyway these materials without written permission from Awake the Soul, LLC.
MISCELLANEOUS PROVISIONS
If any paragraph herein or part thereof is deemed to be invalid or unenforceable for any reason, the remaining paragraphs shall remain in full force and effect.
This Agreement shall be interpreted according to the laws of the State of Colorado.
GAZE MATRIX METHODÂ (GMM)
You may be enrolled in a GMM group, retreat or individual session. Additional terms and conditions apply.
- Participant certifies that he/she is not pregnant and does not have epilepsy; a detached retina; glaucoma; uncontrolled high blood pressure; cardiovascular disease and/or irregularities including but not limited to prior heart attack or heart arrhythmia; prior diagnosis by a Medical Professional of bipolar disorder or schizophrenia; strokes, TIAs, seizures or other brain/neurological condition or disease; family history of aneurysms; use of prescription blood thinners such as Coumadin; hospitalization for any psychiatric condition or emotional crisis within the last ten years; osteoporosis that is serious enough whereby intense movement could cause physical injury; recent physical injuries that are not fully healed and could be re-injured through intense movement; or any other medical or physical conditions which would impair or affect his/her ability to engage in any activities that involve physical and/or emotional release or which would cause any risk of harm to Participant, other participants and/or any participating Breathwork Facilitators and/or Apprentices or otherwise endanger Participant’s health while participating in a GMM session organized by Facilitator or Annie L. Browning. If you have been diagnosed with PTSD and are currently in therapy, it is required that you get the approval of your therapist before participating. Also, if you have asthma, please make sure to have your inhaler next to you when you breathe in case you need it. If you have any doubt about whether you should participate, consult your physician or therapist as well as a Facilitator before attending. Participant also certifies that if he/she has asthma that he/she has brought and will have access to his/her inhaler during the GMM session.
- Participant is aware that certain activities he/she may engage in during the GMM (the “Session”) are physically, emotionally and/or mentally stressful. Among other processes, it will include breathing that is faster and 2 deeper than normal over a prolonged period which can cause dizziness, palpitations, tingling/numbness of the extremities, carpopedal spasms (involuntary contractions of the muscles of the hands and feet), ringing/roaring in the ears, clouded/distorted vision, perceptual distortions, and feelings of lightness, astonishment and/or euphoria. Physical Activities can include violent twisting and turning while sitting or sitting and other exertions not normally engaged in by the Participant. Soft music is also used with the described activities.
- Participant certifies that he/she is at least 18 years of age.
- Participant understands that a Session is intended as a personal growth experience and should not be used as a substitute for psychotherapy.
- Participant agrees to comply with the stated and customary guidance and norms of participation in GMM. If, however, Participant observes any unusual hazard during his/her participation including but not limited to any physical symptoms that Participant feels could be a danger to his/her physical well-being, participant will immediately remove himself/herself from participation in GMM and bring such to the attention of the Facilitator forthwith.
- Participant knows, understands and acknowledges that the Facilitators are NOT physicians, psychologists, or healthcare professionals, and that GMMÂ is NOT intended to treat or diagnose, and does NOT include treatment for or diagnosis of, any illnesses, disease or disorders, whether physical, mental, psychological or emotional.
- Participant knows, understands and acknowledges that the information available on the www.annielbrowning.com website (“The Website”) is intended for informational purposes only. It is not intended to diagnose, mitigate, treat or cure any disease or condition. No claims are made by Facilitator as to specific health benefits. Individuals should consult a qualified health care provider for medical advice. The user assumes all responsibility and risk for the use of the information on The Website. Organizer disclaims all responsibility and liability for any information, practices, or services obtained based on viewing the content of The Website. The content of The Website should not be relied on as being comprehensive, error-free or suitable for any purpose whatsoever. The content included on The Website has not been evaluated or approved by the FDA.
- Participant acknowledges, understands, and agrees that this Agreement, and all of the releases, terms and conditions contained herein, shall apply with equal force and govern any future GMM Programs, both in-person and online, in which participant partakes with Facilitator thus obviating the need for me to sign this Agreement every time I partake in any GMM activities or events. Participant further agrees to bring any changes in their Medical condition to Organizer’s attention before participating in any GMM event.
- Participant understands that since his/her experience will be guided by his/her own psyche, despite any representations made by any of the Facilitators or any marketing materials that Participant may have seen regarding GMM, Facilitators cannot guarantee any specific type of experience, result or benefit from participating in the GMM Program.
- Participant agrees to assume full responsibility for his/her own physical, emotional and mental health and hold harmless Facilitator, and/or any Facilitators and Apprentices working with organizer at the Program from any physical, emotional and/or mental damage that may be attributed to it. Participant further holds harmless organizer from any and all loss, liability, injury, damage or cost which may arise out of or in connection with participation in the Program.
- Participant acknowledges that he/she has been advised (a) concerning the types of activities which will be engaged in during the Session, and understands the risks and difficulties that may arise during the Session; and (b) that if Participant feels too uncomfortable to continue at any point during the breathwork, he/she is to stop immediately and check in with the Facilitator before continuing. Participant understands that he/she may decline to do any of the activities. Participant understands that by executing this release and engaging in the Program, he/she is assuming those risks which are inherent to the activities involved. Participant further understands that these activities are best done in presence of or in conjunction with the Organizer, for best results.
- Participant understands that contra-indications may exist for the anticipated activities if Participant is or has been suffering medical or psychology Participant understands that contra-indications may exist for the anticipated activities if Participant is or has been suffering medical or psychological / psychiatric conditions requiring professional care; and that the activities described can also trigger suppressed traumas.
- Participant agrees to keep confidential any comments, shares or experiences by other participants.

My Commitment to Privacy
Awake the Soul, LLC is strongly committed to maintaining the privacy and security of confidential personal information and other highly sensitive data that it collects. My privacy and public records obligations are governed by applicable state and federal law.

The Way I Use Information
Client information is used to route the requested Web page to your computer for viewing. In theory, the requested Web page and the routing information could be discerned by other entities involved in transmitting the requested page to you. I do not control the privacy practices of those entities. Essential and nonessential technical information helps me respond to your request in an appropriate format (or in a personalized manner) and helps us plan website improvements. Optional information enables me to provide services or information tailored more specifically to your needs or to forward your message or inquiry to another entity that is better able to do so and also allows me to plan website improvements.
I may keep client information indefinitely after the Web page is transmitted, but I do not try to obtain any information to link it to the individuals who browse our website.Â
I use the information you provide about yourself when placing an order or request only to complete that order or request. I do not share this information with outside parties except to the extent necessary to complete that order or request. We generally I return email addresses only to answer the email I receive. Such addresses are not used for any other purpose and are not shared with outside parties.

Providing Information Is Your Choice
There is no legal requirement for you to provide any information at my website. My website will not work without routing information and the essential technical information, however. Failure of your browser to provide nonessential technical information will not prevent your use of our website but may prevent certain features from working. For any optional information that is requested at the website, failure to provide the requested information will mean that the particular feature or service associated with that part of the Web page may not be available to you.
My Commitment to Data Security
To prevent unauthorized access, maintain data accuracy and ensure the correct use of information, I have put in place appropriate physical, electronic and managerial procedures to safeguard and secure the information I collect online.
